ABSTRACT

Objective: To evaluate the experience of caries and tooth loss in rural citrus workers in Northeast Brazil. Methods: This was a cross-sectional, quantitative, and descriptive study carried out using a non-probabilistic sample of rural citrus workers linked to the Occupational Health Reference Center. Generalist and dental questionnaires were applied and clinical examinations were performed by two calibrated examiners on 619 participants in Basic Health Units to establish the sample DMFT index and to detail their characteristics of interest. Results: The sample was predominantly male (71%), with young adults between 20 and 40 years (50,9%), with low education (76%) and low income (59,3%). The mean DMFT index of the sample was 14.29 and the component regarding tooth loss (M) 9.97, corresponding to approximately 70% of this value. Moreover, 90,1% of the evaluated participants had losses of at least one dental element and 27,8% had a functional loss with less than 21 teeth in the oral cavity, while 86.3% said they had never attended an oral health service. Of the participants who have already attended, 83,6% were over six months ago, motivated by pain (43,8%) and caries (14,7%). Conclusions: The rural citrus workers evaluated had negative experiences with dental caries and a high rate of functional tooth loss, leading them vulnerable to stomatognathic system imbalances and showing the need for prevention and promotion of oral health


Objetivo: Avaliar a experiência de cárie e perda dentária de trabalhadores rurais da citricultura no Nordeste brasileiro. Métodos: Trata-se de um estudo transversal, quantitativo e descritivo, realizado com amostra não probabilística de citricultores rurais vinculados ao Centro de Referência em Saúde do Trabalhador. Foram aplicados questionários (generalista e odontológico), e exames clínicos foram realizados por dois examinadores calibrados em 619 participantes em Unidades Básicas de Saúde, para estabelecer o índice CPO-D da amostra e detalhar suas características de interesse. Resultados: A amostra foi predominantemente masculina (71%), com adultos jovens entre 20 e 40 anos (50,9%), com baixa escolaridade (76%) e baixa renda (59,3%). O índice CPO-D médio da amostra foi de 14.29, e o componente relativo à perda dentária (P) de 9.97, correspondendo a aproximadamente 70% desse valor. Além disso, 90,1% dos participantes avaliados tiveram perdas de, pelo menos, um elemento dentário, e 27,8% tiveram perda funcional com menos de 21 dentes na cavidade oral, enquanto 86,3% afirmaram nunca ter frequentado um serviço de saúde bucal. Dos participantes que já frequentaram, 83,6% foram há mais de seis meses, motivados por dores (43,8%) e cárie (14,7%). Conclusões: Os citricultores rurais avaliados apresentaram experiências negativas com cárie dentária e alto índice de perda funcional dos dentes, tornando-os vulneráveis aos desequilíbrios do sistema estomatognático e evidenciando a necessidade de prevenção e promoção da saúde bucal.

ABSTRACT

This study assessed the bond strength of a resin cement when Immediate Sealed Dentin (ISD) was contaminated by different temporary cements applying a microshear test. Forty-eight fragments of third molar crowns were mounted in acrylic resin and worn until the dentin was exposed. The substrates were sealed with a 3-step conventional adhesive system and divided into four groups (n=12): Control (without temporary cement) and three groups that used different temporary cements - Provy, RelyX Temp NE, and Provitemp. The temporary cements were removed after 7 days of storage. Resin cement RelyX ARC was applied to the contaminated substrates. Microshear bond strength was evaluated using a universal testing machine (0.5 mm/min). Data was analyzed with ANOVA and a Dunnett post-hoc test (p<0.05). Additionally, three samples from each group were prepared for scanning electron microscope analysis. Means and standard deviations of microshear bond strength (MPa) were 17.96±1.65, 16.70±1.43, 20.43±1.30 and 33.75±4.72 for Control, Provy, RelyX Temp, and Provitemp, respectively. Statistically significant difference was observed between the Control and Provitemp samples (p=0.0002). Adhesive failures were dominant (50%), followed by cohesive failures with cement (26%), and cohesive failures with dentin (24%). Residues of Provy and Provitemp were observed on the sealed dentin. Provitemp temporary cement sealing increased the microshear bond strength of the resin cement on dentin treated with IDS.


Este estudo investigou a influência de cimentos temporários na resistência de união de um cimento resinoso em dentina selada imediatamente. Quarenta e oito fragmentos obtidos das coroas de terceiros molares foram incluídos em tubos de PVC com resina acrílica e desgastados até exposição da dentina. Os substratos foram tratados com adesivo convencional de três passos e divididos em quatro grupos: Controle (sem cimento temporário) e três grupos usando diferentes cimentos temporários ­ Provy, RelyX Temp NE e Provitemp. Após sete dias, os cimentos foram removidos e o cimento RelyX ARC aplicado sobre o substrato contaminado. A resistência de união foi avaliada em uma máquina de ensaio universal (0,5 mm/min). Os dados foram analisados pelos testes estatísticos ANOVA e Dunnet (p<0,05). Três amostras de cada grupo foram preparadas para análise em micróscopio eletrônico de varredura. Os valores médios e desvio padrão da resistência de união foram 17,96 ±1,65, 16,70 ±1,43, 20,43±1,30 e 33,75 ±4,72 para Controle, Provy, RelyX Temp e Provitemp, respectivamente. Diferença significativa foi observada entre os grupos Controle e Provitemp (p=0,0002). Com relação ao padrão de fratura, a maioria das falhas foram adesivas (50%), seguida por coesiva em cimento (26%) e coesiva em dentina (24%). Resíduos dos cimentos Provy e Provitemp foram observados na dentina selada. Cimento temporário Provitemp aumentou a resistência de união por microcisalhamento do cimento resinoso a dentina com IDS.

ABSTRACT

This study aimed to evaluate the cytotoxicity of intracanal medications on L929 fibroblast cells at different periods of observation. The following experimental groups were studied: calcium hydroxide with camphorated paramonochlorophenol and glycerin (CPG); iodoform with glycerin (IG); calcium hydroxide with iodoform and distilled water (CIW); iodoform with distilled water (IW); calcium hydroxide with distilled water (CW); Otosporin ® (OT); and a control group composed of cells and culture medium. Eluates were prepared from each group and placed in contact with 1 x 105 cells/well for periods of 30 minutes, 12, 24, 48 and 72 hours, 5 and 7 days. After each experimental period, a cytotoxicity test was performed using methyltetrazolium (MTT) and a spectrophotometer at an optical density of 570 nm to analyze cell viability. The ANOVA and Tukey test with a significance level of 5% was used to analyze the data. At 30 minutes and at 12 hours, all groups were equal to the control group. At 24 hours, there was greater cytotoxicity in the IG group than in the control group (P<0.001). At 48 hours, only the OT group was cytotoxic (P <0.001). At 72 hours and at 5 days, the most cytotoxic groups were CW and OT. At 7 days, the IW and CPG groups were the least cytotoxic (P <0.001). With respect to experimental time, significant differences between 24 hours and 7 days were observed in all groups. Otosporin® was the most cytotoxic medication, followed by calcium hydroxide with distilled water.


Este estudo teve como objetivo avaliar a citotoxicidade de medicações intracanais em células L929 de fibroblastos em diferentes períodos de observação. Os seguintes grupos experimentais foram estudados: hidróxido de cálcio com paramonoclorofenol canforado e glicerina (CPG); iodofórmio com glicerina (IG); hidróxido de cálcio com iodofórmio e água destilada (CIW); iodofórmio com água destilada (IW); hidróxido de cálcio com água destilada (CW); Otosporin® (OT); e um grupo controle composto por células e meio de cultura. Os eluatos foram preparados a partir de cada grupo e colocados em contato com 1 x 105 células/poço, por períodos de 30 minutos, 12, 24, 48 e 72 horas, 5 e 7 dias. Depois de cada período experimental, um teste de citotoxicidade foi realizado utilizando metiltetrazólio (MTT) e um espectrofotômetro a uma densidade óptica de 570 nm para analisar a viabilidade celular. A análise de variância e o teste de Tukey com nível de significância de 5% foi utilizado para analisar os dados. Em 30 minutos e em 12 horas, todos os grupos foram iguais ao grupo controle. Em 24 horas, houve uma maior citotoxicidade no grupo IG do que no grupo controle (P<0,001). Em 48 horas, apenas o grupo OT foi citotóxico (P<0,001). Em 72 horas e em 5 dias, os grupos mais citotóxicos foram CW e OT. Aos 7 dias, os grupos IW e CPG foram os menos citotóxicos (P<0,001). Com relação ao tempo experimental, foram observadas diferenças significativas entre 24 horas e 7 dias em todos os grupos. Conclusão: Otosporin® foi o medicamento mais citotóxico, seguido de hidróxido de cálcio com água destilada.

ABSTRACT

Introdução: O cimento resinoso dual é o principal material para cimentação de cerâmicas. Contudo, fatores, como fonte de luz, podem influenciar no desempenho do cimento. Objetivo: Avaliar a influência de duas fontes de luz de diferentes intensidades sobre a microdureza de um cimento resinoso quando fotoativado sob diferentes cerâmicas. Material e método: Foram fotoativados 40 espécimes de cimento resinoso dual (10 mm × 1,0 mm) por meio de duas fontes de luz LED, com intensidades de 1.000 mW/cm² e 800 mW/cm² durante 40 segundos, sob cerâmicas (15 mm × 2 mm) à base de zircônia, espinélio MgAl2O4 e dissilicato de lítio. No grupo controle, foi utilizada uma placa de vidro de mesma dimensão. As amostras tiveram a superfície regularizada com lixa d`água (600 e 1200). Após armazenagem a seco no escuro (24 h), foram realizadas as medidas de microdureza Vickers (HMV Shimadzu - 50g/10 segundos). Os dados foram submetidos à Anova fatorial e ao teste de Tukey com nível de significância de 5%. Resultado: O cimento polimerizado apresentou valor médio de microdureza e desvio padrão para o grupo controle de 43,9±1,5 e 43,4±1,3, para as intensidades de 1.000 mW/cm2 e 800 mW/cm2, respectivamente. O menor valor de microdureza do cimento foi observado para a cerâmica à base de espinélio MgAl2O4, polimerizado a 800 mW/cm², com valor de 34,3±3,3.Conclusão: O tipo de cerâmica e a intensidade da fonte de luz influenciam na microdureza do cimento. A intensidade de 1.000 mW/cm<sup>2</sup> sobre as cerâmicas à base de zircônia e espinélio MgAl2O4 resultaram na melhor combinação de valores de dureza do cimento.<hr/></p> <p>Introduction: The dual-cure resin cement is the main material for luting of ceramic. However, factors such as light source can influence the performance of the cement. Objective: Evaluate the influence of two light sources of different intensities on the microhardness of a resin cement when cured under different ceramics. Material and method: Forty samples of dual resin cement were photoactivated by two LED light sources, with light intensity of 1000mW/cm² and 800mW/cm², respectively (40 seconds), under the base ceramic Zirconia, Spinel MgAl2O4 and Lithium Disilicate. In the control group, a glass plate was used. Each samples had the surface grounded (600 and 1200). After dry storage in the dark (24 hours), Vickers microhardness measurements were performed (HMV Shimadzu - 50gf/10 seconds). The data were submitted to factorial ANOVA and Tukey's test with significance level of 5%. Result: The average hardness and standard deviation in the control group for cement polymerized 1000mW/cm² was 43.9±1.5 and 800mW/cm² the value of 43.4±1.3. The lowest value was for the Ceramic base of spinel MgAl2O4, light cured with 800mW/cm² with a value of 34.3±3.3. Conclusion: The ceramic type and intensity of the light source influence the hardness of the cement. Para isto, procedimentos cirúrgicos plásticos, clareamento dental e trabalho restaurador direto foram propostos. Um melhor contorno gengival com altura adequada de coroa clínica foram alcançados através do procedimento cirúrgico plástico. O clareamento dental e o procedimento restaurador proporcionaram um resultado estético bem agradável. Os procedimentos integrados conseguiram alcançar um resultado satisfatório a paciente e aos profissionais com estética final adequada para a condição clínica<hr/></p> <p>A smile with properly aligned and contoured teeth became a beauty standard of nowadays society. Therefore, many patients search for dental treatment in order adjust their oral and facial condition in accordance to these beauty standards. However this kind of goal may not be achieved immediately requiring prior treatment plan that includes a multidisciplinary team. The present article reports the case of a young patient who wanted to modify her smile pattern. The suggested treatment included plastic surgeries, dental whitening, and direct restorative work. A better gingival contour with adequate clinical crown height was achieved through plastic surgery. Teeth bleaching and restorative procedure have provided a very satisfactory aesthetic result. Forty dentin discs had thick (TSL) or thin (THSL) smear layer created on their occlusal side. In artificial pulp chambers, MDPC-23 cells were seeded on the pulpal side of the dentin discs and divided into five groups: G1TC: no treatment (control); G2TC: TSL + RMGIC; G3TC: THSL + RMGIC; G4TC: TSL removal + RMGIC; G5TC: THSL removal + RMGIC. After 24 h, cell metabolism and morphology were evaluated by the methyltetrazolium (MTT) assay and by scanning electron microscopy (SEM), respectively. For BS, the following groups were determined: G1BS: TSL removal + RMGIC; G2BS: THSL removal + RMGIC; G3BS: TSL + RMGIC; G4BS: THSL + RMGIC. Shear bond strength was tested to failure in a mechanical testing machine MTS (0.5 mm/min). Statistically significant difference was observed only between the control and experimental groups (Kruskal-Wallis, p<0.05). The metabolic activity of the viable MDPC-23 cells in G2TC, G3TC, G4TC and G5TC decreased by 54.85%, 60.79%, 64.12% and 62.51%, respectively. Mean shear bond strength values for G1BS, G2BS, G3BS and G4BS were 7.5, 7.4, 6.4 and 6.7 MPa, respectively, without significant difference among them (ANOVA, p>0.05). RMGIC presented moderate transdentinal cytotoxic effects. Maintenance or removal of smear layer did not affect the bond strength of RMGIC to dentin substrate.<hr/></p> <p>Este estudo avaliou a citotoxicidade transdentinária (CT) e a resistência de união (RU) de um cimento de ionômero de vidro modificado por resina (CIVMR) aplicado sobre dentina coberta com smear layer (SL) de diferentes espessuras. Quarenta discos de dentina tiveram smear layer espessa (TSL) ou delgada (THSL) criadas sobre a superfície oclusal. Após terem sido posicionados em câmaras pulpares artificiais, os discos de dentina receberam células MDPC-23, as quais foram semeadas sobre a supefície pulpar. Assim, os seguintes grupos foram estabelecidos: G1TC: sem tratamento (controle); G2TC: TSL + CIVMR; G3TC: THSL + CIVMR; G4TC: remoção TSL + CIVRM; G5TC: remoção THSL + CIVMR. Após 24 h, o metabolismo e morfologia celular foram avaliados pelo ensaio de metiltetrazolium (MTT) e por microscopia eletrônica de varredura (MEV), respectivamente. Para BS, os seguintes grupos foram determinados: G1BS: remoção TSL + CIVRM; G2BS: remoção THSL + CIVRM; G3BS: TSL + CIVRM; G4BS: THSL + CIVMR. A resistência de união ao cisalhamento foi avaliada em uma máquina de ensaios mecânicos MTS (0,5 mm/min). Diferença estatisticamente significativa foi observada apenas entre os grupos controle e experimentais (Kruskal-Wallis, p<0,05). A redução da atividade metabólica das células MDPC-23 viáveis nos grupos G2TC, G3TC, G4TC e G5TC foi de 54,85%; 60,79%; 64,12%; e 62,51%, respectivamente. Os valores médios de resistência de união para G1BS, G2BS, G3BS e G4BS foram de 7,5; 7,4; 6,4; e 6,7 MPa, respectivamente, sem diferença significativa entre eles (ANOVA, p>0,05). O CIVMR avaliado neste estudo apresentou moderado efeito citotóxico transdentinário.
